Role Description

This is a full performance level Professional Registered Nurse who provides nursing services and conducts various types of assessments for chronically ill and elderly residents.

  • Conducts home visits to assess the health care needs of individuals.
  • Identifies services for individuals who are at risk of Nursing Home Placement.
  • Serves a population that includes infants, youth, adults, and seniors.

Examples of Work

  • Provides direct nursing services to individuals utilizing the nursing process by conducting a comprehensive evaluation to determine medical needs and establishing a Person-Centered Plan of Care.
  • Teaches and demonstrates care procedures and health promotion concepts to clients and their families.
  • Documents assessments, treatment, intervention, and progress in agency medical records in accordance with Maryland Department of Health (MDH), Health Department (HD), and Federal guidelines.
  • Confers with health professionals, Supports Planners, family members, and other designated individuals on the care needs of clients.
  • Completes weekly/monthly narrative and data reports.

Qualifications

  • Graduation from an accredited school, college, university or an equivalent program with a bachelor's degree in nursing or a related health field or an equivalent combination of education and experience.
  • Possession of a valid license to practice as a Registered Nurse in the State of Maryland issued by the Maryland State Board of Nursing at the time of appointment.
  • Possession of a valid license to operate a motor vehicle when required for the performance of job-related duties at time of appointment.
  • Possession of a valid cardiopulmonary resuscitation (CPR) certification.

Preferred Qualifications

  • 2 years of home-visiting experience.
  • Proven experience and desire to work in a team-oriented, fast-paced, and professional environment.
  • Ability to communicate effectively, orally and in writing.

Conditions of Employment

  • Meet all training and performance standards and demonstrate proficiency as required by the agency.
  • Wear and use agency protective apparel and equipment in the performance of their assigned duties, if applicable.
  • Successfully pass preemployment checks which may include reference checks, background investigations, and drug screenings, where applicable.
  • Be willing and able to serve as an essential employee.
