RN Home Health Assessor (MLTC) Hybrid Remote

About the position

The Registered Nurse (RN) Home Health Assessor position at Maximus involves conducting UAS assessments to determine eligibility for community-based long-term services within the New York Independent Assessor Program. This hybrid remote role requires a dedicated RN with experience in home health settings, focusing on delivering accurate assessments through telehealth or in-home visits. Responsibilities

  • Review favorable and partially favorable determinations in accordance with applicable regulations.
  • Render medical necessity determinations for assigned cases.
  • Resolve technical issues within reconsideration assignments.
  • Review assigned cases or sites to summarize facts and assess identified issues.
  • Perform special projects such as legal and medical research, drafting proposals, or acting as a liaison for specific projects.
  • Conduct initial assessments of consumers, evaluating health status, functional status, strengths, care needs, and preferences.
  • Review consumer medical documentation and health referral forms as relevant to cases.
  • Enter assessment data into the UAS-NY online application and transmit as required.
  • Code UAS assessments correctly based on training and guidelines.
  • Document outcomes of calls to consumers and scheduled appointments.
  • Maintain knowledge of regulatory and project changes related to assessments.
  • Enhance communication and collaborative relationships with leadership and interdisciplinary care team members.
  • Review non-qualified assessments with the Clinical Quality Assurance Department.
  • Maintain a comprehensive working knowledge of community resources and network services for the target population.

Requirements

  • Active RN license required.
  • Minimum 2 years clinical experience required with RN license.
  • Minimum of 2 years' home setting experience, including work with CHHAs, LHCAs, MLTC, or as a paraprofessional (e.g., HHA, LPN, or Private Duty Nursing).
  • Active and valid RN license in the state of New York.
  • Ability to travel within assigned region, including to consumer homes.
  • Active and valid New York State Driver's License or ID.
  • Ability to conduct in-home health assessments throughout the assigned region.
  • Proficiency in navigating web-based applications, Microsoft Outlook & Word, and conducting assessments via Zoom.
  • Knowledge of nursing theory, state, and agency regulations.
  • Strong organizational, interpersonal, and communication skills.
  • Ability to work with multi-generational consumers and demonstrate sensitivity in a fast-paced environment.
  • Capability to manage multiple complex tasks and work independently.

Nice-to-haves

  • Experience with telehealth assessments.
  • Familiarity with UAS-NY Community Health Assessment Reference Manual.
