Social Worker/ Case Manager (Bilingual in Mandarin)

About the position

Responsibilities

  • Perform client intake by phone or walk-in.
  • Determine eligibility and service needs by taking information from clients or collateral sources.
  • Screen for benefits and entitlements, assist with applications and advocacy for government and privately funded programs.
  • Assist clients in accessing services and provide referral linkages.
  • Conduct Person-Centered Options Counseling based on client preferences for services.
  • May conduct in-home client visits to assist with entitlement and benefits applications and assess clients' status.
  • Utilize a database system to maintain complete and accurate case records.
  • Compile statistics, prepare reports, and disseminate information as necessary.
  • Establish and maintain community outreach and perform public education presentations.
  • Work with the Program Director in program planning and development, including coordination of special programs or events.
  • Provide task-based assistance to new Specialists as needed.
  • Complete annual mandatory training on various topics including compliance and regulatory standards.

Requirements

  • Bachelor's degree required for Case Manager; MSW required for Social Worker.
  • Bilingual in Mandarin is required.
  • Working knowledge of Microsoft Office Suite and other technology.
  • Excellent communication and listening skills.
  • Strong customer service skills with a focus on treating clients with respect and dignity.
  • Ability to multi-task and work independently with great attention to detail.
  • Minimum of 2 years of experience in case management.
